New / Amended Employee Detail Form …


Use :

To be completed by all new employees to enable a payroll record to be set up.  

All information gathered will be used in the processing of the payroll and any returns/forms submitted to HMRC

P46 Form …


Use :

Section 1 to be completed by any new employee who does not have a P45 form from their previous employer.


Completed forms should be forwarded to RMD Accountancy

Self Certification Form …


Use :

To be completed by an employee after a period of sickness of 4 consecutive days or more.  

NB : Doctors notes are required for any sickness extending beyond 7 days.
